B. Briefly describe projects, grants, programs and/or activities in which you have participated that have benefited science education in Wisconsin at the district/local, regional, and/or state level. (One page limit)
C. Provide evidence of your involvement in professional activities. Describe and explain your memberships in professional organizations. Include years of participation, offices held, presentations/in-services given, conference involvement and other activities that demonstrate leadership in your field at the district/local, regional and/or state levels (One page limit).
D. Two letters of support (pdf files may be a good idea)
--Secure two letters of support citing evidence of your contributions to science education. One of these letters should be from a supervisor in your building.
